Cooperation of 3 hospitals for the sake of saving one patient
Our news is dedicated to the cooperation of 3 hospitals to save one patient who was hospitalised in the 8th hospital with gastrointestinal bleeding in a very serious condition. The patient had a history of surgery. After a previous pancreatic necrosis, a pancreatojejunoanastomosis was performed. After gastroscopy and colonoscopy, the source of bleeding was not established, so a video capsule endoscopy was prescribed to examine the small intestine. A.O. Mylyanovska, a proctologist and endoscopist at the Kyiv Regional Clinical Hospital, Proctology Department, performed video capsule endoscopy in the intensive care unit of the hospital's 8th intensive care unit. The images showed a large blood clot 1 minute after the video capsule entered the small intestine. That is, the source of bleeding was most likely located in the area of the ileal anastomosis. Since conservative therapy was not effective, and surgery was extremely risky, it was decided to refer the patient to the National Institute of Surgery and Transplantation named after A.A. Shalimov for angiography with embolisation. The patient successfully underwent embolisation of the splenic and gastroduodenal arteries. The bleeding was stopped and the patient was discharged home in a satisfactory condition.
